Phase 2: Full Program Launch and Cohort Execution (Months 7–30)

Month 7–12: Cohort 1 (100,000 Participants) – Full Launch

  • Key Milestone: Official start of the Celebrate Health program with Cohort 1, encompassing 100,000 participants across all five NYC boroughs.
    • Action Items:
      • Launch the first cohort of 100,000 participants and ensure support across boroughs via HQ and satellite locations.
      • Start the physical training and workshops for participants.
      • Ongoing media campaign to highlight success stories and build momentum for Cohort 2.
      • Track participant progress (health outcomes, engagement) and adjust logistics as necessary.

Month 13–18: Program Execution and Mid-Cohort Adjustments

  • Action Items:
    • Continue delivery of physical training, workshops, and educational content.
    • Monitor participant health and make program adjustments based on feedback (both participant and staff).
    • Continue the media exposure to promote positive results, recruit participants for Cohort 2, and ensure program visibility.
    • Adjust training materials and delivery methods based on feedback from Cohort 1.

Month 19–24: Cohort 1 Graduation and Cohort 2 Enrollment

  • Key Milestone: Cohort 1 graduation and Cohort 2 launch.
    • Action Items:
      • Host a graduation ceremony for Cohort 1 at Month 19 and track long-term health improvements.
      • Begin Cohort 2 enrollment and planning for the next cohort’s execution.
      • Track the impact on NCDs and continue to fine-tune program delivery.
      • Increase the scale of satellite locations if necessary to support growing numbers of participants.
      • Continue media campaigns with a focus on success stories from Cohort 1.

Month 25–30: Cohort 2 Graduation and Preparing for Scaling

  • Action Items:
    • Gradually move participants from Cohort 2 into the final phase of the program.
    • Host graduation ceremonies for Cohort 2 and assess overall impact.
    • By Month 30, the program should be ready for scaling but no nationwide rollout yet.
    • At the conclusion of Month 30, 100,000 participants would have completed the program, and success data will be compiled for national expansion.
    • Prepare a detailed report on program effectiveness, participant health improvements, and lessons learned for use in national scaling.
